What is the Frailty (FAA) consultation template?

The Frailty template records a Rockwood score, Holistic Needs Assessment and onward referrals in one consultation, coded straight back to the patient record.

📖 Learning: The Frailty template is built to the ICB Facilitation of Admission Avoidance (FAA) service specification, maintained by the MLCSU Data Quality Team. Like every consultation in Joy, each answer carries a SNOMED code and is sent to the patient record.

The template is split into sections you can move between as the conversation goes:

  • Overview: Rockwood Clinical Frailty Scale, frailty coding, Reasonable Adjustment Digital Flag, cohort register and care coordinator

  • Carer and Next of Kin: carer status, fall-back arrangements, key holder and key safe

  • Observations: height, weight, BMI, blood pressure and pulse, plus lifestyle: diet and MUST score, exercise, alcohol and smoking

  • Holistic Needs Assessment: domestic abuse safety check, social isolation and loneliness, coping at home, home safety, benefits, continence, sight and hearing

  • Mental State, Legal, DNACPR and ReSPECT: mood, PHQ-9, memory assessment, lasting power of attorney, mental capacity, resuscitation status and advance care planning

  • Care Plans, EuroQoL Assessment, Onward Referrals and Medication Review

💡 Hint: Please record the Rockwood score and the Holistic Needs Assessment on the same day — the service level agreement requires it. If you change a Rockwood score, save the template straight away and then reopen it, so the frailty coding section for levels 5 to 8 appears.

💡 Hint: Medication Review should only be completed by a qualified prescriber. If that is not you, please pass it to your practice nurse, pharmacist or GP.
