How do I discharge a client?

This article shows you how to discharge a client, including how to send this information back to their clinical record.

How to discharge a client

  1. Select the client 🧑 you would like to discharge by selecting their name on the 'Clients' tab

  2. Head over to the ‘Profile’ tab

  3. Under ‘Client status’ select ‘Edit’ 👇

  4. Click ‘Active’ 👇 and scroll down in the drop-down menu to find ‘Discharged - successful’ (or - other, or - no show as appropriate), then click on it 👇

  5. Select ‘Save’

  6. You can then write in any comments on the discharge ✍️, and you can enter their star rating of the service

  7. If you would like to send this back to the clinical record tick the 'Save to clinical record' box ☑️ shown below

  8. You will then be given an option to add a SNOMED code along with the discharged action, if you would like to do this click 'Add SNOMED Codes' 👇 as show below and search for the code you would like to add:

  9. Choose the codes you would like to add by searching for them, and ticking the tick box next to them ☑️, then select 'Add' 👇

  10. Click 'Save'

  11. You have successfully discharged a client! 🎉


Understanding average annual GP appointments data

📖 Learning: Once a client has been referred to Joy, Joy counts the number of GP appointments the client has had in the previous 6-months based on clinical system data. Following discharge, Joy will count the number of GP appointments the client has in the next 6-months. These can be compared to understand how social prescribing has reduced the amount of GP appointments needed!

This information will automatically show up, so sit back and relax!
