How to discharge a client
Select the client case🧑 you would like to discharge by selecting the clients name on the 'Client Cases' tab
Select the case you need to discharge and on the overview page of the case click on the dropdown option of the status
Click ‘Active’ 👇 and scroll down in the drop-down menu to find ‘Discharged - successful’ (or - other, or - no show as appropriate), then click on it 👇
You can then write in any comments on the discharge ✍️, and you can enter their star rating of the service
If you would like to send this back to the clinical record tick the 'Save to clinical record' box ☑️ shown below
You will then be given an option to add a SNOMED code along with the discharged action, if you would like to do this click 'Add SNOMED Codes' 👇 as show below and search for the code you would like to add:
Choose the codes you would like to add by searching for them, and ticking the tick box next to them ☑️
Click 'Save' ✅
You have successfully discharged a client! 🎉
Understanding average annual GP appointments data
📖 Learning: Once a client has been referred to Joy, Joy counts the number of GP appointments the client has had in the previous 6-months based on clinical system data. Following discharge, Joy will count the number of GP appointments the client has in the next 6-months. These can be compared to understand how social prescribing has reduced the amount of GP appointments needed!
