Open up a patient 🧑 on either SystmOne or EMIS

Open up JoyConnect by clicking the '+' button 👇 next to 'Joy'

Click 'Refer' 👇 underneath the service you would like to refer the patient to ​ ​The majority of the referral form will be filled in for you by the clinical system

Ensure you have checked they meet the service criteria and then tick this box ☑️

Select the reasons for the referral ☑️

You can also optionally add any additional needs ☑️ the patient may have (for example, they may be partially sighted)

Click the tickbox ☑️ to confirm the service provider can directly contact the client.

Click 'Send referral' ​ ​Everything will be added back onto the clinical record with the appropriate coding should you press 'Edit' to include SNOMED codes

Click 'Done' or 'View referral' 👇 to see the completed referral form

You have now sent a referral using JoyConnect! 🎉
