Open up a patient 🧑 on either SystmOne or EMIS
Open up JoyConnect by clicking the '+' button 👇 next to 'Joy'
Click 'Refer' 👇 underneath the service you would like to refer the patient to
The majority of the referral form will be filled in for you by the clinical systemEnsure you have checked they meet the service criteria and then tick this box ☑️
Select the reasons for the referral ☑️
Add in any referral notes ✍️
You can also optionally add any additional needs ☑️ the patient may have (for example, they may be partially sighted)
Click the tickbox ☑️ to confirm the service provider can directly contact the client.
Click 'Send referral'
Everything will be added back onto the clinical record with the appropriate coding should you press 'Edit' to include SNOMED codesClick 'Done' or 'View referral' 👇 to see the completed referral form
You have now sent a referral using JoyConnect! 🎉
