Every client profile in Case Management has a Wellbeing tab. It holds two things: the wellbeing questionnaires (called trackers) you have recorded or sent for that client, and the Concerns you are tracking with them over time.
This guide is a tour of the screen, so you can read it confidently before you start adding anything.
Open the tab
In the Case Management sidebar, open Client cases.
Open the relevant client.
Select the client's Wellbeing tab.
The Wellbeing trackers card
The Wellbeing trackers card is the main part of the tab, with a New tracker button in its top right corner.
If nothing has been recorded yet, the card simply says No wellbeing trackers yet.
Once there is something to show, the trackers are grouped into one block per framework. A block is everything you have collected with that questionnaire for this client: the first measurement and every follow-up after it.
Each block shows:
the framework name at the top, for example ONS4, PAM or WEMWBS, with an Archived badge beside it if the block has been archived;
a summary line reading Baseline 12 , latest 16 (+33%) — the first score, the most recent score, and the change between them as a percentage. The percentage is green when the score has moved in the better direction and red when it has not. For frameworks where a lower score is the better result, the percentage is followed by the word change, for example (-20% change);
for PAM only, an extra line reading PAM level: baseline 2, latest 3;
the buttons Follow-up, Change framework and Archive (or Unarchive on a block that is already archived).
Reading one tracker row
Underneath the summary, each measurement in the block gets its own row. Reading a row from left to right:
Baseline or Follow-up — whether this was the first measurement or a later one.
The status: Completed, Cancelled or Awaiting. Awaiting means it has been sent to the client and they have not filled it in yet.
How it was done: In person, Email, SMS or WhatsApp.
Either Score 16 for a completed tracker, or Sent 4 Aug 2026 for one that is still awaiting the client.
A completed tracker that has been sent to the client's GP record also shows a clinical-record badge. It reads Sending to clinical record while it is queued, then Sent — awaiting confirmation, then Filed to clinical record when it lands. If something goes wrong it reads Sending failed, and Sending cleared if the failed attempt has been cleared.
A tracker that was captured inside a consultation shows a From consultation badge, usually with the template name and date, for example From consultation: Frailty (FAA) (12 Jul 2026). Select it to open that consultation.
Under a row that is still Awaiting, a small grey line tells you where the automatic reminders have got to, for example Day-3 reminder sent 7 Aug 2026 · next reminder due 11 Aug 2026. It can also read Reminders stopped 8 Aug 2026, Automatic reminders off (organisation setting), or No further automatic reminders.
The Concerns card
Below the trackers is the Concerns card, with a New concern button. A concern is a worry or need in the client's own words that you rate from 0 to 10 over time, where 0 means resolved and 10 means as bad as it gets.
Each concern shows its title, any description, its need-category badges, and a summary line reading Baseline 8 , latest 3 (improved by 5). If nothing has been recorded, the card says No concerns recorded yet.
Sections some organisations see
Above the ordinary Wellbeing trackers card, some organisations also see:
a WorkWell trackers card, for organisations taking part in the NHS WorkWell pilot;
a Youth Guarantee Trailblazer card, for organisations taking part in that programme.
These are switched on per organisation. If you do not see them, your organisation is not on those programmes and there is nothing to set up.

Charts of wellbeing outcomes across all your clients are on the Dashboard, under Outcomes — this tab shows one client at a time and has no charts.
