Where your organisation is connected to the GP clinical system, a completed wellbeing tracker can be written back into the client's GP record with SNOMED CT codes, so the score is visible to their clinical team.
There are two routes: decide up front while you record or send the tracker, or file a result you have already collected. Both are below.

Route 1: decide when you record or send the tracker
In the Case Management sidebar, open Client cases.
Open the relevant client and select the Wellbeing tab.
Start the tracker as usual — New tracker, or Follow-up on an existing block.
Near the bottom of the dialog, tick Save to clinical record.
Leave Destination on Detect automatically unless you need to choose a particular system for this client.
Check the SNOMED CT codes. The framework's own code is filled in for you when it has one; search for and add any others you need, and remove any you do not.
Finish as normal with Record or Send.
An in-person tracker is queued for the clinical record as soon as you select Record. A tracker you sent is queued only when the client completes it — nothing is filed while it is still Awaiting.
Route 2: file a result you already have
Find the completed tracker row on the client's Wellbeing tab.
Select File to clinical record on that row.
The dialog confirms what is going: "Send this ONS4 result (score 16) to the client's GP record with the SNOMED codes below."
Check the SNOMED codes, again pre-filled with the framework's own code where it has one.
Select Send to clinical record.
You will see Queued for the clinical record, and a status badge appears on the row.
Reading the status on the row
Sending to clinical record — queued and waiting to go.
Sent — awaiting confirmation — it has gone to the clinical system, which has not confirmed yet.
Filed to clinical record — it is in the client's GP record. Nothing more to do.
Sending failed — it did not go through. Tell whoever looks after your clinical integration; the failure is recorded with the reason.
Sending cleared — a failed attempt has been cleared and will not be retried.
Notes and things that can stop you
Each result can only be filed once. If it is already queued or filed, the File to clinical record button is not shown on the row, and trying again gives you Already queued — "This result is already queued for the clinical record."
Only Completed trackers can be filed. There is nothing to send while a tracker is still Awaiting the client.
Nothing is filed unless someone asks for it: leaving Save to clinical record unticked, and never using File to clinical record, means the result stays in Case Management only.
Filing depends on your organisation being connected to a clinical system. If the Destination list is empty, or results sit at Sending failed, ask whoever manages your clinical integration to check the connection.
Concerns and their ratings cannot be filed to the clinical record — this is for wellbeing trackers only.
