Proactive Care

Written by Nico Campodonico

You can use Proactive Care to identify patients who may benefit from support and decide whether to initiate outreach.

Before you start

Proactive Care must be enabled for your organisation and role. The cited guide describes point-of-care review when a patient record is opened, not advance scanning of full patient lists. A recommendation supports, but never replaces, clinical judgement.

Step by step

  1. Open Proactive Care in JoyConnectWeb.

  2. Review the patients in Triage: these are opportunities not actioned during the consultation.

  3. Select a patient, verify their identity and review why they were flagged and the suggested services.

  4. Use your judgement to choose an appropriate available action: enrol, open suggested services, send selected services, review more detail or archive an inappropriate case. Confirm contact details and consent before contacting the patient.

  5. Review Cases (also called All Cases) to check what has already been actioned.

  6. To track a service, open Services and click Add a Service to save the listing. Review referral counts, outcomes and top referrers there.

  7. Open Outcomes (ROI / Outcomes) to review available before-and-after information. Treat missing or insufficient data as unavailable, not as evidence of improvement.

Check the result

You understand why a patient was flagged, have chosen an appropriate next action, and know where to check completed actions. Missing or recent outcome data is not proof of an effect.
