  1. Go to the 'Clients' tab

  2. Then select the client 🧑 by clicking on their name

  3. Then go to their 'Client record' tab, under their name

  4. From here, record the contact or note you made ✍️, such as details of a recent interaction by filling out the popup that appears in the right-hand box on the screen

  5. After you’ve created the note, scroll down and click 'Send to Clinical Record' 👇

  6. Ensure you choose the correct clinical record for the patient (this should be the same as their GP practice)

  7. Then select 'Save'

  8. The clinical system should look like below when selected (turned purple)

  9. Select 'Add CT SNOMED Codes' to add relevant SNOMED codes (Optional), this will open a pop-up in which you can search for relevant codes, tick next to the appropriate code, then select 'Add' 👇

  10. Record Relevant Data ✍️, you can also record how much time you spent with the client (for example, if you spent 15 minutes on the call, enter that), you can also record the time and date of the note, and upload any files you may want to add

  11. Once everything is filled in, scroll to the top and click 'Save and Send' 👇, you’ll be asked if you're sure you want to send the note to the clinical record—click 'Yes' 👍 to confirm

  12. That’s it, your note will now be integrated into the patient’s clinical record! 🎉
