💡 Hint: These custom questions will appear under additional referral details within the received referrals (either within the case management software or via email).
In your browser go to https://services.thejoyapp.com/and login
Hover on the letter in the circle in the top right-hand corner 👇, usually, this will be the first letter of your initials
Once you hover over this, you should see a number of options open up like this image below
Select the option that says ‘My Listings’ 👇
This will bring up all of the listings that you are currently active, select the listing 👇 where you’d like to update the referral form (in this example, it’s the bereavement service training)
Clicking on the title of the listing 👇 will load the listing page, this is all information that is currently shown on Joy, but with the choice of additional options to editing at the right-hand side as shown below
Please find and select the ‘Manage service referral form’ 👇
Select ‘Edit forms’ 👇
Choose which questions you would like to be required fields (grey means it is not required, blue means it is), required means it will not allow the referral to be submitted without this information
You can also manage which questions appear on self-referral versus professional referral forms by checking (and unchecking) the appropriate options under the question
If you would like to add a custom question, select ‘Add custom question’ 👇 next to ‘Additional referral details’
From here a new blank question will appear
If you want to add custom answers for your questions you can click on the area of the form next to the check box (for checkboxes), circle (for multiple choice), or lines (for dropdowns), and write them in
To edit a previous custom question that you have clicked off of, you simply need to click on the question again
Once you have added all the questions you would like, hit ‘Save and publish’ ✅
Congratulations, you have edited your referral form and added custom questions! 🎉
