How do I refer into my own service?

This article and video walks you through how to refer into your own case management software.

πŸ’‘ Hint: This can be used to create a client in the case management software, for example, if you are transferring your caseload on to Joy.

  1. Open up the case management software (case.thejoyapp.com) and log in using your details
    ​

  2. Select the left-hand 'Services' tab πŸ‘‡ (not the one within a client record)
    ​

  3. Select 'My organisation's services' πŸ‘‡ tab and locate the relevant service (for example, if you are a social prescriber, find the social prescribing listing)
    ​

  4. Select 'Refer' πŸ‘‡ on the relevant service (the one which your team uses, this will usually be the name of your PCN and then the team your role falls under for example: Example PCN - Social Prescribing)
    ​

  5. It will now open a new page in the Marketplace
    ​

  6. On the page that opens select 'Make a referral' πŸ‘‡
    ​

  7. Select 'I am referring someone else' πŸ‘‡
    ​

  8. Fill in the referral details ✍️, if you have the NHS spine active you can enter their NHS number (or full name and DOB) under 'Look up client' and this will also pull through their information into the referral form
    ​

  9. Press 'Submit' πŸ‘‡
    ​

  10. Return to case.thejoyapp.com, and accept the client from the 'Inbound referral' tab as you usually would
    ​

  11. You have now transferred this patient into Joy! πŸŽ‰
