How to add a follow-up and see a client's progress

A wellbeing tracker only shows a difference once you measure the same questionnaire a second time. That second measurement is a follow-up, and it goes into the same block as the first one so the two can be compared.

Add a follow-up

  1. In the Case Management sidebar, open Client cases.

  2. Open the relevant client.

  3. Select the client's Wellbeing tab.

  4. Find the framework block you want to add to, and select Follow-up on it.

  5. The Add follow-up dialog opens: "Record a follow-up in person, or send it for the client to complete."

  6. Choose How to complete:

    • to fill it in together, choose In person, answer the questions on screen, and select Record;

    • to let the client answer, leave it on Email (or choose SMS or WhatsApp), pick the address or number under Send to email / Send to number, and select Send.

An in-person follow-up shows Follow-up recorded and appears immediately as a Completed row. A sent one appears as an Awaiting row with its sent date, and behaves exactly like a sent baseline — same reminders, resend and stop-reminders controls, all covered in How to send a wellbeing tracker to a client.

Read the client's progress

Once there are two or more completed measurements in a block, the summary line under the framework name tells the story:

  • Baseline 12 is the first score you recorded.

  • latest 16 is the most recent one.

  • The figure in brackets, for example (+33%), is the change between them. Green means the score has moved in the better direction, red means it has not. On frameworks where a lower score is the better result, the figure reads for example (-20% change).

  • On PAM, the extra line PAM level: baseline 2, latest 3 shows the activation level as well as the score.

Every measurement stays listed underneath, oldest first, so you can talk the client through each step rather than just the headline. There are no charts on this tab — for wellbeing outcomes across your whole caseload, use the Outcomes tab on the Dashboard.

Tidy up a block

  • Change framework moves the whole block to a different questionnaire: "Move this tracker block to a different framework." Choose the framework and select Change. You will see Framework changed. Use this only when the block was started on the wrong framework.

  • Archive hides a block you have finished with. It stays on the tab with an Archived badge, and the button becomes Unarchive so you can bring it back.

  • To remove a single measurement, select the red bin icon on its row. You will see Deleted. There is no undo, so check you have the right row first.

Notes

  • A completed tracker cannot be edited. If a score is wrong, delete that row and record it again.

  • Deleting the last remaining row does not delete the block — use Archive to put a finished block out of the way.
